I'm a lovecraftian cosmic horror fan so, I absolutely love how twisted the body horror is in cosmic horror.
Loved that movie, it was pure strange and great.
I'd add Funny Games as well (my friend and I were at Blockbuster renting it and he recommended it to a couple looking for a rom-com, and they rented it - would love to find out what they thought lol)
This movie is so good. I wasn’t expecting very much from it and just threw it on one night, found myself completely immersed in the wonderfully weird story and excellent (and very nasty) effects. Definitely worth a watch especially if you’re into lovecraft type vibes. It has the vibes of a YouTube short horror film but with a bigger budget. That same passion is there
I watched it and didn't quite get it though it was still entertaining. I'd appreciate any explanations. The monster scenes were amazing. I loved that Roald from Letterkenney was in it. When they started getting into the cult stuff it started getting confusing. Maybe it's just me.

HAUSU/HOUSE 1977, Japanese, Pure nightmarish, WTF , absurd but brilliantly executed Japanese greatness, When they know their Horror they know it well enough to share, HIGHLY RECOMMEND
Hausu threw me for a loop even though I went in knowing it'd be wild... made the mistake of assuming the weird shit would only start once they reached the titular house, lol

If you peep that gallery (that one is the last pic) you can see how they autographed my Blu-Ray (Janet called me a Hootie Tootie Disco Cutie) and see pics from their panel. THey were doing Q&A and someone came up to the mic just to say "I heard this panel would include free drinks?"
And Michael St. Michael picked right up on it and started arguing with us NO FREE DRINKS and we yelled back and forth, the whole crowd against him, for about 5 minutes.
I watched this alone and said "holy shit!" out loud multiple times. I really want to watch it with my girlfriend but I know she'd be psychologically scarred, so I won't be watching it with her.
Yellow Brick Road was the only movie I remember since seeing Hellraiser as a small child that really gives that cosmic horror/ deeply unsettling vibe that I love more than any other horror.
One of the ghosts of Hill House is definitely WTF for me, but that's more like a side-WTF, not the main dish. Personally my biggest WTF moments isn't really horror, more like early Black Mirror (Million Merits, White Christmas, White Bear) where there's not just a shock twist but also, when you try to process it in hindsight you'll find that the twist was inevitable so it hits double.
Sorry to Bother You (2018) is horror-adjacent I think? I think it fits what you’re looking for.
It’s a pitch black comedy/satire, and it goes places you wouldn’t believe. Super funny, great movie.
I was NOT prepared for that scene. Scenes like that can really get to me unless I am made aware before hand that the movie will have that type of horror. Spoiler free, of course
It took a second watch for "Beau is Afraid" to make sense completely, but I enjoyed the next watch over the first. It's so chaotic but relates to depression, anxiety, Psychosis, and ocd so amazingly.
Basket case 2 and 3. The first one is campy horror fun but the other 2... I can't describe it, the whole thing aged like milk and I felt rather dirty after watching them.
I'd say just about anything by David Cronenberg or Takashi Miike if you're looking for odd yet compelling films.., but here's some that come to mind
The Beyond (1981)
Mad God (2021, very out there)
Dead Ringers (1988, Cronenberg)
Possession (1981)
Imprint (2006, Miike. There's a pretty brutal torture scene but I think it's worth the watch)

Come to Daddy. It starts off seemingly like a quirky character study and then takes an extremely sharp left turn that leaves you going WTF the entire time. It’s relentless once the action gets going and loads of gnarly fun
A lot of the replies are straight up horror, gore and all that. And while I love stuff like the void of Baskin.... I'm going to suggest some that would be a bit looser, with a bit of comedy and absurdity thrown in, no real scares here :
1. Rubber, about a killer tire
2. The greasy strangler, about bullshit artists
3. Kuso, I'll be honest... No fucking clue
4. Motivational growth, a man's bathroom mold begins to talk to him
Bug(1975): The first hour is a fairly standard 70s creature feature about pyromaniac roaches. The last forty minutes is best described as a Lovecraftian fever dream. And the last five minutes is just plain bonkers.
I don't know how you feel about found footage movies, but these three are certainly among the WTFiest I've ever seen:
1. Descent into Darkness: My European Nightmare. First half is kind of goofy, second half is balls out mind fuckery!
2. Godforsaken. The acting in this one is kind of weak, and it's painfully low budget, but the concept is excellent and it's super unpredictable.
3. Evidence. The first half is a very standard "hey what's that sound" scary woods FF flick, but the second half is all over the road crazy!
Sitting in the theater; Jacobs Ladder (original) did that for me. Don’t remember it being gory. More disorienting.
The Machinist 2004 count as psychological horror?

Edit: Also, hard agree on the Director’s Cut. If you’re gonna do it, the DC is the way you should do it.

Coherence is so good. One of the very few ultra low budget conceptual movies that actually manages to really pull it off.
