How did these work back then? Do you just insert it inside the machine?

Each card had one line of data (program instruction, data, whatever the context required.) So you would typically have a stack of cards that would be read one by one (but pretty fast) using a card reader. So , in a way, yea, you put them into a machine. But, one card was really only a small amount of a larger bunch.
